Where Does the Money Go?
You decide how you would like us to use
your gifts. In general, donations can be made in support of
our Chapter efforts in Arizona and southern Nevada, providing
programs and services locally, or you can specify that you wish to
support research efforts. We send donations designated for
research to our National office in Chicago. Either way, it is
very helpful for us to know what your intentions are when sending in
your contributions.
There is a third option, which allows
donations designated to research to qualify for discounts on the
amount of dues the Chapter pays monthly to the National Association.
It's called the Dues Offset Program.
Research Donations and the Dues Offset Program
Our National Association is by far the largest
private funding source for Alzheimer's disease in the country.
In fact, to date they have invested more than $185 million into more
than 1,400 projects. Their grants program has launched the careers
of some of the most stellar scientists in the field, responsible for
major discoveries in our understanding of the disease.
Additionally, many of those researchers are working right here in
Arizona and southern Nevada.
We send all research-specific donations to our
National office in Chicago, but in case you'd also like to
support our local efforts to provide critical services, there is a
solution that benefits all of us--it's called the Dues Offset
Program.
As part of our affiliation agreement, we pay
monthly dues to the National office. In turn, they support our
local efforts on a national scale. You can help us reduce the
amount of dues we are required to pay by designating your
research-specific gift to the Dues Offset program. In this
way, 100% of your gift will be applied to Research, PLUS, a
percentage of that total will be subtracted from our monthly dues.
Everybody wins!
Here's an example: A $1,000 gift for
research sent to our Chapter Office will be forwarded to Chicago.
If the donor also requests that the gift qualify for Dues Offset, an
additional $400 will be discounted from the dues we are
required to pay--so your $1,000 gift has a $1,400 value. (For
tax purposes, however, only $1,000 is deductible!)
We've listed below some of the different ways
in which you can choose to support us--whichever way you may choose,
please know that we are grateful for your support as we move ever
closer to our vision of a world without Alzheimer's disease.
Categories of Giving
Annual Giving Program - a yearly donation made in
support of local programs and services.
Matching Gift Program - employer-sponsored matching gifts
programs enable you to increase your contribution to the Chapter or your Region.
Memorial Contributions - a donation made in memory of a
deceased family member or friend.
Tributes - a donation made in honor of a living person or
special occasion, such as an anniversary, birthday, wedding, etc.
Donations of Appreciated Property - a way of decreasing your
tax burden by donating unsold stock to the Chapter and claiming a deduction for the value
of the stock on the day you contribute. Proceeds from the sale of the donated stock will
benefit the Chapter or Region, whichever you choose.
Planned Giving - a means of including the
Desert Southwest Chapter in
estate planning, through a bequest, gift annuity, charitable remainder trust, or as the beneficiary
of a life insurance policy.

Planned Giving
A bequest is the most common
form of planned gift, and is a living testimony to your endorsement of our vision of a world
without Alzheimers disease. You help us fulfill our mission. Bequests to charities are not subject to inheritance and estate taxes. What
better way to preserve the quality of life for individuals coping with Alzheimers disease than to remember the Alzheimers
Association Desert Southwest Chapter in your will. All it
takes is a simple amendment (codicil) to your will. Below is sample bequest language you can take to your attorney or financial advisor:
"I give, devise, and bequeath _______
(insert amount of gift, percentage of estate or residuary of estate) to the
Desert Southwest
Chapter of the Alzheimers Disease and Related Disorders Association, Inc., 1028 East
McDowell Road, Phoenix, Arizona 85006."
If you have already included us in your
will, or are planning to do so, we thank you.
There are many creative ways to help us support those in our
Chapter community who are suffering from Alzheimer's disease. Each way has a special benefit to the donor - this simple chart outlines some ways to give a gift.

Winston Churchill once wrote, "We make a living by what we get. We
make a life by what we give."
Why Support the Alzheimer's Association?
Today over 100,000 individuals in Arizona and southern Nevada are coping with the devastating effects of
Alzheimers disease. Currently 10% of the population over the age of 65 have
Alzheimers disease and nearly 50% of those over age 85, the fast growing segment of our
population. In the next 20 years, these numbers will almost double.
The National Alzheimer’s Association is the single largest private donor
toward research efforts to eradicate this disease. This is important to all of us who
look forward to a world without Alzheimers. The focus of the
Desert Southwest Chapter,
however, is on the local community of affected individuals, family members and
caregivers.
Alzheimers not only destroys the lives of those who have the disease, it also
exhausts the spirit and depletes the savings of families who care for their suffering
loved ones. The Desert Southwest Chapter of the Alzheimer's Association has been helping caregivers
for over 20 years. When we are able to connect, we know we make an extraordinary
difference. Our services and programs provide information and support, which helps to
alleviate the stress experienced by family caregivers. When you make a gift to the
Desert Southwest
Chapter, you enable us to continue to connect with families in many ways.
Our five Core Programs are at the heart of the efforts we make every day.
Here is a description of those programs. When you decide to support our
Chapter, you may designate one or more of these Core Programs, or you may
signify that your gift is designated for wherever it will do the most good.
Our telephone Helpline and Information Referral is the lifeline to the community. Trained
volunteers link callers with resources they need, share critically needed information as
well as provide a compassionate and sympathetic ear, 24 hours a day, 7 days a
week. The Helpline provides
information on all aspects of this disease from specifics on the disease itself, to
how to provide an appropriate home environment for someone with Alzheimers, to legal
and financial planning. The Helpline also makes appropriate referrals of
physicians, nursing and assisted living facilities, attorneys, research programs and much
more
Knowledge is power. That is why Educational Workshops are so important.
Individuals diagnosed with Alzheimers and their families can participate in our
training programs and learn about the disease, its stages and effective interventions.
Caregiver
Support Groups as well as early-stage patient groups build on
relationships and caring and help reduce the sense of isolation many caregivers
experience. They are moderated by trained volunteers who can help
participants understand and cope with the sometimes devastating issues
associated with the disease.
One-on-one counseling can be be provided through
Family Care Consultation to those either unable to afford the services of private geriatric
care managers and/or those not yet financially eligible to receive public case management
and long-term care services.
The vast majority of people with Alzheimer's disease will, at some point
during the course of their illness, wander away from home and loved ones.
Safe Return® is a 24-hour nationwide identification, support and
enrollment program.
All of our programs and services are made possible through generous charitable
contributions from friends in our community. All our funds come from individuals like you, from
corporation and foundation grants, and from our special events. There is so much more that
we could be doing. With your help we will continue to offer our programs and services and
increase the number of people we serve. Your continued generosity will help make that
critical difference in the lives of many Alzheimers families in our community.
There are other ways to make a gift to the Alzheimers Association
Desert Southwest
Chapter, such as life income gifts, gifts of real estate, and gifts of life insurance. Find
the gift that works for you and make your gift to the Alzheimers Association. For
more information, contact Sandy Dominick, Development Director, at 602-528-0545 or

All contributions are tax deductible according to IRS
statutes. We are a nonprofit organization under IRS 501 (c) (3)
guidelines Upon receipt
of your contribution, a thank you letter and receipt will be sent to you for tax purposes.
HOW TO LIVE FOREVER...The
Founder's Society!
Our Desert Southwest Chapter unveiled its Founder's Society, and we offer
you an opportunity to become a member. Your membership in this honorary society
brings special privileges and benefits. If you wish to join, call Sandy Dominick at 528-0545 for
more information.
The Founder's Society is an association of friends of the Alzheimer's Association
Desert Southwest Chapter, who ensure victory over Alzheimer's disease by way of deferred gifts and
philanthropy. To become eligible for membership, you need only to inform us in writing
that our Chapter has been included in your estate plan.
Charter members play a unique role as their participation will motivate and encourage
others to participate. Because it secures the future of the Alzheimer's Association
Desert Southwest Chapter, it brings you a special kind of immortality. We think that's a legacy
everyone will want to leave.
MEMORIAL GIVING
The life of each person who has been touched by a spouse, father, mother, friend,
neighbor, etc. who has or had Alzheimer's disease is never the same. Each person is
motivated in different ways to help eliminate this disease and help ease the burden of
caregivers. Many make contributions to research or to support the Chapter. Some volunteer
their time and talents. Many others do both. Some host events such as birthday,
anniversary or retirement parties, golf tournaments, garage sales, etc. and give the
proceeds to the Alzheimer's Association. Many encourage friends and family members to
support the Association with gifts in lieu of flowers upon the death of their loved ones.
The note below typifies a request for help in doing just that: "I need some help. My
mother, who is loved very much, is in the final stages of Alzheimer's disease.
Unfortunately, my siblings and I need to start getting things in order for her passing. My
mother would have wanted a cure to be found so nobody else would ever suffer and she would
have wanted family caregivers to receive the help and support they need to take care of
their loved ones suffering from this disease. When her time comes, instead of receiving
flowers and cards from family and friends, we think it would be better if they made a
donation to the Alzheimer's Association in her memory. How can we make that happen? Thank
you so much." This question frequently arises when families are faced with the
ultimate death of their loved one. The family may be strongly motivated to help in the
work to find a cure and to continue caregiver programs and services. One of the easiest
ways to make that happen is to include a sentence in the obituary of their loved one that
states the following:
"In lieu of cards and flowers, please send a donation to the Alzheimer's
Association Desert Southwest Chapter,
1028 E. McDowell Road, Phoenix, AZ, 85006."
By placing such wording in the obituary, families are able to create a memorial to
their loved one and at the same time provide support to the Alzheimer's Association.
The family might take this one step further and establish a Memorial Fund with the
Alzheimer's Association in memory of their loved one. The
Desert Southwest Chapter has instituted
the establishment of a new special category of giving. A Memorial Fund will be established
with a contribution totaling $500 to honor the memory of a person. The initial
contribution may be made by an individual, a group, an organization or a corporation. Any
person may add to an established Memorial Fund at any time. An Honorary Fund in honor of a
person or special occasion for a living individual can also be established with a
contribution of $500 as a tribute to honor a person.
Memorial gifts tend to be one-time gifts. Next month a special mailing will be made to
past memorial donors asking them to consider making another gift in memory of their
special someone. If you have been a one-time memorial donor, we hope you will become a
regular donor. Your gift will contribute toward continuing the much-needed support for
caregivers.
If you have questions about listing in an obituary or creating a special Memorial or
Honorary Fund, please call Sandy Dominick at (602) 528-0545.

LOOK FOR THE LOGO!
Many organizations use the word "Alzheimers" in
their names. However, only the Alzheimers Association can use the distinctive logo,
a design combining an intertwined symbol of caring for people and
supporting science. This logo depicts the two distinct roles the
Association serves in the lives of the community: HELP for patients and
caregivers through Chapter programs and services and HOPE through supporting Alzheimers
research leading to a world without Alzheimers.
If you receive a mailing from someone soliciting funds for
Alzheimers, be sure to look for the Alzheimers Association logo and
our local
Phoenix street address, which is included on all correspondence from our chapter. We do not share
names or sell our mailing list to any organization. We also do not have any control over
mailings you may receive from the Alzheimers Association National Office in Chicago.
Also, our Chapter does not conduct phone solicitation campaigns.
